I am over the age of 18

I will abstain from drinking alcohol and caffeine for 24 hours prior to my treatment.

I have declared any medications / contraindications that may affect my cosmetic tattoo treatment

I am fit and well for my treatment to take place

I understand that all booking deposits and treatments are non refundable

Failure to attend a scheduled appointment or give 72 hours notice to reschedule you will forfeit your deposit.

A scheduled complimentary touch up appointment must be taken within 8 weeks of the first appointment, this will be booked at the initial appointment and should not be changed. Bianca will book these touch up appointments as a priority, when rescheduling it is difficult to fit in within the complimentary window. If a scheduled complimentary top up appointment is missed or cancelled we will be able not offer an additional free appointment. An appointment will then be made at a cost to you.

I understand that the provider of this procedure takes no responsibility for any possible complications and consequences that may result from the procedure, particularly if I neglect to answer the pre-treatment medical form properly, failure to accurately disclose my medical history or fail to take pre-procedure &/or aftercare advice.

I understand that all permanent make up procedures carry with them the possibility of complications and consequences including but not limited to fading of skin pigments, risk of infection, scarring, inconsistent colour and bruising.

A patch test is not necessary however it will be completed on the day of your treatment.

I understand that a skin patch test can determine if I will have a reaction to the products but that it is inconclusive regarding whether I will have an allergic reaction at any time in the future.

If I have a history of sensitivities or past reactions and would prefer to have a patch test prior to my treatment I will inform Bianca J Cosmetics who can arrange this.

I understand I require a follow up visit (perfection visit) in 6-8 weeks if I am to get the best results from the procedure. This must be booked within 3 months of my first visit. Failure to do so may results in paying more.

I understand that some skins do not hold pigment at all and therefore results cannot be guaranteed. If additional treatments are needed to achieve the look I require this is at an extra cost to me.

I understand that whilst every effort is made to ensure symmetry, I understand that no ones face is symmetrical. The artist do her utmost to ensure a symmetrical tattoo but this can be unachievable at times.

I accept that the highest standards of hygiene are met, and that sterile disposable needles are used for each individual client, procedure and visit.

The general nature of tattooing as well as the specific procedure to be performed will be / has been provided and explained to me and my treatment will not start until I agree.

I understand that cosmetic tattooing is a process with healing variables, therefore healed colour cannot be guaranteed.

I understand that it is a multi-treatment process with colour being implanted slowly and carefully over a period of time in a layering process.

Aftercare has been / will be discussed with me before my treatment, detailed aftercare will be given to me to take home and it is my responsibility for my aftercare. If not carried out properly this can result in additional fading, risk of infection or scarring.

My chosen colour will look darker when implanted but will lighten and soften over the next 7-14 days.

I understand that any additional work cannot be completed for a minimum of 4 weeks to allow the skin to heal fully.

I understand that all colours will fade and alter with time. To keep a fresh appearance a maintenance/retouch appointment is required. Fade is dependant on age, skin type, lifestyle, medications and sun exposure.

I understand that if I have an MRI or CAT scan, I must tell the radiologist that I have had cosmetic tattooing as you may experience a slight tingling in the affected area.

If I am to donate blood following my permanent make up treatment I am to inform the National Blood Service as I am unable to donate for 1 year post my treatment.

I am aware that sun exposure, future skin altering procedures, such as plastic surgery, peels, implants, injectables may alter the appearance of my procedure.

I acknowledge that when providing the information in this form I consent to the use of this information for the purpose of provision of services by Bianca J Cosmetics.

I consent to the photographs, video or other images of myself being utilised by Bianca J Cosmetics on social media.

I consent to the uploading of my personal information to a cloud computer network.
